What boxes are checked on the CCF if the donor was unable to provide a specimen?

Urine and Oral Fluid Specimen Collections

  • In the “Collection” section, located in Step 2 of the Custody and Control Form, the collector should ensure the “None Provided” box is checked
  • The collector must also enter a relevant remarks about the collection attempts in the Remarks section.
    • Remarks should always include information about each collection attempt made and the time each collection attempt (i.e. 1st attempt 1:30 pm QNS; 2nd attempt 2:30 pm QNS; 3rd attempt 4:30 pm QNS), and the amount of water the donor consumed (i.e. donor drank 40 oz of water)
    • QNS stands for “quantity not sufficient”.
  • Because a specimen was not collected, there’s nothing to send to the laboratory and the collector should shred copy one of the CCF and send copy 2 to the MRO and copy 4 to the employer.
