Non-DOT Alcohol Test Form
Use the Non-DOT Alcohol Test Form to record results for any alcohol test that is not a DOT regulated test. The non-DOT alcohol test forms look identical to DOT alcohol test forms, except all references to the U.S. Department of Transportation have been removed from the text. DOT regulations specifically state that non-DOT alcohol tests should not be recorded on DOT alcohol test forms. The Non-DOT form should be used for breath or saliva alcohol tests that are not covered under the DOT testing program.
The non-DOT alcohol test forms can be used to record any non-DOT alcohol test result regardless of the testing methodology (breath or saliva), and regardless of instrument type. These non-DOT alcohol test forms are specifically designed to accommodate instruments that use the Slip Printer to print results directly on the form, as well as the Alco-Sensor IV @Work software. They can also be used with any breath test device that prints results that are to be fastened to a form.
