When doing a DOT specimen collection when is it appropriate to check the “Other” box for reasons for test on the Federal CCF form?

The “Other” box is strictly for the USCG when conducting “Periodic” collections. The other DOT agencies do not recognize any other reasons for testing other than, Pre-employment, Random, Reasonable Suspicion, Return-To-Duty, Post-Accident and Follow-Up. EX: Recertification is not a viable reason to test an employee under DOT regulations. If drug testing under a recertification exam is required by the employer, the specimen collector must conduct the test as a non-DOT test, using a non-Federal form.
